1. Upload the dd-contact-form folder to the /wp-content/plugins/ directory
2. Activate the plugin through the ‘Plugins’ menu in WordPress
3. (not essential) You can set the form’s settings via the WordPress Settings Menu -> DD Contact Form
4. Place shortcode [dd_contact_form] on the pages / posts where you want the form to appear
